    About This Career Opportunity

    This position will involve 80% care coordination and 20% Corporate projects and administration.


    This position provides care management services to members of the Community with the goal of assuring that appropriate health and psychosocial services are made available to and utilized by individual members. This position is responsible for recommending, developing, implementing and supervising care programs for individuals identified by the Community which assure sound professional practices, provide safe and pleasant environments, respect the individual’s preferences while meeting their needs, and reflect the values of the Community.  This position will work with the designated Community

    liaison(s) to develop and implement a cost-efficient system of care.  This position is also responsible for establishing and maintaining effective communications with, its members, members’ families, and other individuals and/or organizations providing care and services.   

    Are You Qualified?

    Current licensure as LVN, RN or MSW.


    Three years’ experience working in geriatric case management. 


    Demonstrated knowledge regarding aging issues, dementia care services, knowledge of community resources, and long-term care models. 


    Valid driver’s license and dependable means of transportation. 


    Ability to travel frequently in carrying out job responsibilities, with occasional travel throughout California and other locations as needed.


    Sensitivity and knowledge of working with older adults and the aging process and implications of aging in a social setting.


    Excellent written and verbal communication skills. 


    Ability to identify need, develop and manage care and systems simultaneously. 


    Must understand health care environment and the psychosocial and medical context for older adults, with ability to communicate professionally, empathetically, and compassionately with elder population who may be ill, disabled, hospitalized, and/or experiencing loss.
